I am installing wood base and crown. I have my three coats of stain on for desired color. My question is, do I need to apply a final coat of polyurethane, and what kind and how do I apply it?

I would definitley poly the base boards. I think the crown might be a personal preference. I don't think there is a particular brand of poly you should use but I'd use a fine brisseled brush for the application. I like quick dry poly. I wouldn't get anything high gloss.

what about spray-on?

I've never used spray on before. I don't see why not. I'd put several coats on though. Base boards tend to get alot of abuse. And with spray there is the tendency to miss spots.
