Please see attached picture, it is a new poured slab, is this hole/crack going to be a major problem?

You lost some strength. Probably major depending on where it goes.
Who put the cover-up patch on? - Am I reading the picture right?

Here are two more pictures, it is at the back of the house. The hole is more than 2 inches deep.

That's some sloppy work. And right under the cripple & king of the header too. That point will be taking 2-3x the load of a typical area of the foundation. I'd be pissed.

Ouch! I agree with Vector. At the very least you should insist that the header be redone. You can make it longer so that the weight will be transfered to a soild spot in the foundation. They can then reframe your opening under it.

Builder's solution is to fill the area with concrete and add an extra floor plate, don't know if this will solve the problem.
