In Inland Empire Socal:

What material is used, if any, as a moisture barrier in a slab on grade? Is it just a sheet of plastic of a certain mil thickness?

Is it necessary to put perforated pipes inside the concrete slab below the mositure barrier for drainage? I don't see any in my existing home.

What adhesive (brand, chemical type) is used between the pressure-treated sill plate and the concrete slab?

Typically a 6mil polyethylene barrier is used under slabs. One issue with this is during the finishing - all the bleed water will rise to the surface so be ready for it.

We typically use PL400, a polyurethane product, for adhering PT to concrete. If it's an exterior wall we also use anchor bolts at 32" o.c. Many people will just use sill sealer but it doesn't work very well as a sealer, in my opinion.
